Personal Data Protection Application Form
(IN ACCORDANCE WITH THE PERSONAL DATA PROTECTION LAW NO. 6698)
(REGARDING APPLICATIONS TO BE MADE BY THE PERSONAL DATA OWNER TO THE DATA CONTROLLER1)
GENERAL EXPLANATIONS;
Personal data owners defined as the relevant person in the Law on the Protection of Personal Data No. 6698 (“Law”) (“data owners”) are granted certain rights regarding the processing of their personal data in Article H of the Law.
According to Article 13, paragraph 1 of the Law; applications to be made to our Company, which is the data controller, regarding these rights must be submitted to us in writing or through other methods determined by the Personal Data Protection Board (“Board”).
In this context, applications to be made to our Company in “written” form must be sent to us by printing out this form;
• By the applicant’s personal application,
• Through a notary,
• By signing with the “secure electronic signature” defined in the Electronic Signature Law No. 5070 by the applicant and sending it to the Company’s registered e-mail address.
Below, information is provided specific to the written application channels regarding how written applications will be delivered to us.

In addition, after the other methods determined by the Board are announced, our Company will announce how applications will be received through these methods.
Applications sent to us will be answered within thirty (30) days from the date the request reaches us through one of the methods described above, depending on the nature of the request, in accordance with Article 13, paragraph 2 of the Personal Data Protection Law. Our answers will be delivered to the applicant in writing or electronically in accordance with the provisions of Article 13 of the relevant Personal Data Protection Law.

D. Please select the method by which we will notify you of our response to your application: I want it to be sent to my address. I want it to be sent to my e-mail address. (We will be able to respond to you faster if you choose the e-mail method) I want to receive it by hand. (In case of receipt by proxy, a notarized power of attorney or authorization document is required.) This application form has been prepared to determine your relationship with our Company, to determine your personal data processed by our Company, if any, in full, and to respond to your application correctly and within the legal period. In order to eliminate legal risks that may arise from unlawful and unfair data sharing and especially to ensure the security of your personal data, our Company reserves the right to request additional documents and information (copy of identity card or driver’s license, etc.) for identification and authorization. If the information regarding your requests submitted within the scope of the form is not accurate and up-to-date or if an unauthorized application is made, our Company does not accept liability for requests arising from such incorrect information or unauthorized application. |
